To attach the shelf slide to the wall mount bracket for use with a four slot cradle:

1.

Place a slide on the left-hand bracket, aligning the larger pan-head screw holes in the slide 
with the left two screw holes on the bracket. 

2.

Secure the slide to the bracket by inserting the two pan-head screws provided from below 
the bracket, up through the bracket’s screw holes and then through the slide’s pan-head 
screw holes.

3.

Place a slide on the right-hand bracket, aligning the larger pan-head screw holes in the slide 
with the right two screw holes on the bracket. 

4.

Secure the second slide to the bracket as described in Step 2.
