To add a drop-down list that hat users can use to select a single option:
a) Click Drop-down list.
b) At Group label, enter any text that you want to appear before the
options.
c) At Choice..., enter the name that you want to appear for each option
that users can select.

To change the order of the items on the list, from the form page:

b) Select the item you want to move and then use the up and down
arrows to move it.
