Do you want to use the "Received:" header or a custom header to determine the originating 
IP address?
1. Use "Received:" header
2. Use a custom header

Enter the custom header name that contains the originating IP address:

Choose the operation you want to perform:
- NEW - Create a new entry
- EDIT - Modify an entry
- DELETE - Remove an entry
- PRINT - Display the table
