This chart contains reference information for the patient. To set up the patient chart:

1. 

Select the Main Menu icon on the Navigation Bar.

2. 

Select the Implant button (Figure 22).

Select the Patient View icon to access the Patient View screen.

4. 

The SQ-RX device model and serial numbers appear on the first line of the chart. 

The electrode model and serial numbers appear on the second line of the chart. The 

implant date appears on the third line of the chart (Figure 23). Using the on-screen 

keyboard, enter the following patient information:
